Routine Appointments

To book an appointment:

  • call 01709 584885 between 8:00am and 6:30pm Monday to Friday
  • request an appointment using our Accurx Triage Link between 8:00am and 6:30pm Monday to Friday
  • if you’re unable to call or request an appointment online, visit the surgery in-person between 8:00am and 6:30pm Monday to Friday

Routine appointments are available to book online up to 4 weeks in advance.

Care Navigation is about getting the right care to the right person the first time. We may offer an alternative to a GP appointment if it is suitable for your condition and could mean you are seen sooner.

Book an Appointment Online

Online Booking Tool (Accurx Triage):

Use our Online Booking Tool to request appointments, fit/sick notes, repeat prescriptions, reviews, screenings and vaccinations, read online advice and find local services and self-referral options. No registration is needed, click here to use this service.

SystmOnline:

Use SystmOnline to book and cancel appointments, order repeat prescriptions and view summary record items. To use this service, contact the surgery to be registered and receive a User ID and password. If you are registered, click here to log in to SystmOnline.

NHS App:

Use the NHS App to check symptoms, get instant advice, order repeat prescriptions, view your GP medical record, access your Covid vaccination status and more. To use this service, download the app from your mobile phone’s app store and follow the step-by-step registration instructions.

Urgent Appointments

Same-day appointments are available for urgent acute problems. These are limited and are quickly booked up each day.

To book a same-day urgent appointment:

  • call 01709 584885 from 8:00am Monday to Friday
  • send a request through our Accurx Triage Link from 8:00am Monday to Friday
  • if you’re unable to call or request an appointment online, visit the surgery in-person from 8:00am Monday to Friday

Please note we are not an emergency service. In an emergency, call 999 or visit A&E. Find your nearest Urgent Care Centre.

Cancelling your appointment

If you cannot attend an appointment for any reason, please inform us as soon as possible.

To cancel or change your appointment:

  • call 01709 584885 between 8:00am and 6:30pm Monday to Friday and press Option 2 to cancel
  • use our SystmOnline booking system from 6:40am Monday to Friday
  • visit the surgery in-person between 8:00am and 6:30pm Monday to Friday
  • for certain appointments, you can also click the link in your appointment confirmation text at any time

Due to a large number of appointments being missed each month, we use the following policy:

  • If two appointments are missed without prior cancellation in a six-month period, the patient will be sent a formal warning letter.
  • If a third appointment is missed, the patient’s registration will be discussed with the clinical team which could lead to removal from the practice list and the patient having to find another GP.

Extended Access

Extended Access appointments are available outside of normal surgery hours.

We can book routine GP / ANP / Nurse / HCA / Phlebotomist / Clinical Pharmacist appointments for our patients to be seen:

  • Monday to Friday (weekdays) 6:30pm to 8:30pm
  • Saturday and Sunday (weekends) 8:00am to 12:00 noon

These appointments will be held at either:

  • Broom Lane Medical Centre
  • Kimberworth Park Medical Centre
  • Magna Group: Valley Health Centre and Highthorn Road, Kilnhurst
  • Rawmarsh Health Centre

These can be booked in-person at the surgery or by calling 01709 584885 Monday to Friday from 8:00am to 6:30pm.

These services are not provided by Crown Street Surgery and are the responsibility of South Yorkshire Integrated Care Board, Connect Healthcare (CIC) or Wentworth 1 PCN.

Out of Hours Appointments

If you are calling between 6.30pm and 8.00am Monday to Friday or anytime Saturday and Sunday, please contact the NHS 111 service. To do this, dial 111 from your handset. This call is free from landlines and mobiles. You can also use the 111 website.

In an emergency, dial 999.

Home Visits

Home visits are now routinely carried out by our Home Visiting Team by ACP’s or Paramedics. The service is provided by Connect Healthcare (CIC) and Wentworth 1 PCN (W1PCN).

Home visits are available for the following groups of patients at the clinician’s discretion:

  • terminally ill
  • housebound
  • patients who are severely ill and cannot be mobilised

Requesting a Home Visit

Call the surgery on 01709 584885 Monday to Friday from 8:00am and before 10:30am where possible.

Asking for a home visit does not guarantee you a visit. As home visits are time-consuming, please help us by visiting the surgery whenever possible. GP’s will also have access to your medical records and have better facilities for examining and treating you.
Please note we cannot undertake home visits for patient’s convenience or lack of transport.

What will happen?

Our receptionist will ask the nature of the problem to enable the doctor to decide the urgency of the visit. The doctor will phone you to check the nature of your problem and assess the need for a home visit. It may be that your problem can be dealt with by telephone advice or that is may be more appropriate to send a nurse or arrange hospital attendance. Most home visits are carried out by a Home Visiting Team who will then liaise with the referring GP with any outcomes or treatment plans as required.
