Care Navigation

Care Navigation is about getting the right care by the right person the first time. We may offer an alternative to a GP appointment if it is suitable for your condition and could mean you are seen sooner.

Healthcare Assistant (HCA)

Our HCA’s can conduct clinical checks (such as measuring height, weight and blood pressure), take blood samples, perform ECG’s and basic wound care, review some long-term chronic health conditions, conduct tele-dermatology checks (skin conditions such as moles, warts, verrucae etc), ear syringing and more.

Nursing Team

Our nursing team can do all of the above, as well as provide vaccinations, conduct cervical screening, review medication, perform contraception injections and prescribe oral contraceptives, carry out baby immunisations, conduct NHS Health Checks, offer advice and guidance on menopause and more.

Phlebotomist

We have a phlebotomist available to conduct blood tests when requested by another healthcare professional.

Physiotherapist

Where suitable, patients can be given an appointment with a physiotherapist at the surgery where your condition can be assessed.

Pharmacist

A lot of conditions can be treated by our Community Pharmacists, including medication reviews. Your local pharmacy can also help with a lot of conditions and medication.

Self-Referrals and Local Services

You can now self-refer to a lot of local services without the need for a GP appointment, including:

  • ADHD and Autism Assessments
  • Ear Care and Audiology
  • Continence
  • Speech and Language Therapy
  • Mental Health and Counselling
  • Lifestyle (including weight management, stopping smoking, and drug and alcohol services)
  • Pregnancy, Family and Children’s Services
  • Domestic Abuse
  • Sexual Health
  • Minor Eye Clinic
