Right to Choose Adult ADHD Assessments

Right to Choose is your legal right in England to pick an NHS-funded healthcare provider for your first specialist appointment.

You can now request a referral from your GP to a private ADHD assessment provider. These providers offer NHS services to an NHS standard with NHS prescriptions, if required.

“How do I request a referral under Right to Choose?”

In order to submit a Right to Choose referral, we will require some information from you, including:

  • your full name, date of birth and address;
  • details of your ADHD symptoms and how long you have been experiencing these;
  • a completed questionnaire, known as the ASRS Self-Report Scale (click to view), which assesses whether you meet the criteria for an ADHD referral;
  • the name of your chosen ADHD assessment provider;
  • a completed referral form, where possible.

Approved ADHD assessment providers can be found on the ADHD UK website alongside the approximate wait times for each. Each will have a link to the providers website and many will have a referral form available to complete on their website. If a referral form is not available, please provide the other required information and the name of your chosen provider as we may still be able to process your referral. Please note: we are unable to choose a provider for you.

The quickest way for your referral request to be processed is to send all of the above information in an email to Crown Street Surgery: syicb-rotherham.crownstreetsurgery@nhs.net
If you are unable to send these via email, please bring paper copies into Reception.
We also have paper copies of the ASRS Self-Report Scale available to collect from Reception if required.

“What happens next?”

Upon receipt of all this information and confirmation that the requirements have been met, a referral will be sent and you will be notified.
If you do not meet criteria or we are unable to send a referral to your chosen provider, a member of our team will be in touch to inform you.

“Can I get medication?”

The provider you choose will be responsible for your assessment, medication and follow-up care. It is important when selecting a provider that you check whether they require a shared care agreement from your GP. When looking at providers on the ADHD UK website, you have the option to view more information for each and it will state whether a shared care agreement is required. If it says ‘yes’, then we will be unable to submit this referral as we do not offer shared care.

“Can anyone use Right to Choose?”

In order to use Right to Choose for an ADHD assessment, you must be registered to a GP in England and meet the criteria in the ASRS Self-Report Scale.

You cannot access services via Right to Choose if any of the following apply:

  • Already receiving mental health care following an elective referral for the same condition; or
  • Accessing urgent or emergency (that is, crisis) care; or
  • Accessing services delivered through a primary care contract; or
  • In high secure psychiatric services; or
  • Detained under the Mental Health Act 1983; or detained in a secure setting. This includes people in or on temporary release from prisons, courts, secure children’s homes, certain secure training centres, immigration removal centres or young offender institutions; or
  • Serving as a member of the armed forces (family members in England have the same rights as other residents of England).
